you are an existing patient and would like to fill additional
prescriptions, you do NOT need to fill out these
forms again. Simply FAX your prescriptions
to us and we will process it. Please make sure
your NAME and PHONE NUMBER is clearly
written or printed somewhere on your forms. If you need a
REFILL, just go to the Refills page and
place your order there.)

sign and date forms, attach a copy of both your
prescription and proper identification. |

Prescription
must be issued by a licensed American physician
and must be
current (within the last three months).

A
photocopy of one piece of primary identification
(e.g. driver's license
or passport) OR two pieces of secondary
identification is required for verification
purposes.

you are registered with us, you do NOT need to fill out these
forms again for us to fill additional prescriptions.

orders are shipped by registered, insured mail.
The price of shipping is $11.95 regardless of the number
of medications, size or weight of the package.
Please
allow 15 to 25 days for delivery from time you receive a
telephone confirmation of your order. |
